You don't pay that to the stores, hence the preorder doesn't include it.

While it will run on a 56k modem, don't expect the best :) (goes without saying nowadays really).

As for a free trial, I believe you get a month free with the game.

where are we meant to pay our monthly subscriptions to? I'd fel much happier buying it from MPUK than from some other websit tbh.

The monthly subscription is payable to SOE, as you use their service to play the game.
